It Starts With You Regularly monitor your health
Minimize the cost associated with managing chronic conditions Detect issues early on From buckling your baby’s seat belt to installing safety devices for your children to keeping a first aid kit on hand, you do a lot to take care of your family. Why not do the same for yourself? Living a healthy and productive life starts with taking control of your well-being—and that begins with regularly monitoring your health. Think of it as your early warning system. Keeping track of your health can help you avoid illness and detect problems even before you notice the symptoms—and before they have a chance to get worse. It Starts With You Effectively manage any chronic medical conditions
Chronic diseases cause 7 out of 10 deaths in the U.S. Avoid complications that can affect your health and quality of life—and health care costs In addition, if you do happen to have a chronic disease or condition, such as heart disease, stroke, cancer, type 2 diabetes and obesity, it’s important that you take an active role in managing your condition. In fact, according to the CDC, chronic diseases cause 7 out of 10 deaths in the U.S. each year, as well as higher rates of disability. If you’re not managing your condition effectively, a number of complications may arise that can affect your health and quality of life—as well as your health care costs.

Metabolic Syndrome Is the Key
It’s important to note that not all of these conditions are outwardly obvious. High Blood Sugar (this condition, in and of itself, is an indicator) Low HDL (Known As “Good” Cholesterol) 3 or more of these measures indicate Metabolic Syndrome High Body Mass Index (or BMI) High Triglycerides Among other things, your biometric screening will identify whether you have what’s called metabolic syndrome. Metabolic syndrome is a cluster of conditions, including high blood sugar, high triglyceride levels, increased blood pressure, high body mass index (or BMI) and low HDL (known as “good”) cholesterol. In fact, high blood sugar, in and of itself, is an indicator of metabolic syndrome. It’s important to note that not all of these conditions are outwardly obvious. Someone with high triglyceride levels or low HDL cholesterol may not be aware. That’s where the biometric screening comes in. High Blood Pressure

Metabolic Syndrome Implications

However, any of these conditions increase your risk of serious disease, such as heart disease, stroke and diabetes. And having three or more of these indicates metabolic syndrome, which increases your risk of serious disease even more. In fact, those with metabolic syndrome are seven times more likely to develop diabetes, three times more likely to develop heart disease and two times more likely to suffer a stroke. Not to mention the increased health care expenses, as the out-of-pocket cost for medications for those with metabolic syndrome is typically four times higher than those without. In addition, those with metabolic syndrome tend to lose two more working days per year than those without. Heart Disease 2X Stroke

Think of it as being on an escalator, and your current lifestyle is what’s keeping you moving up the escalator. Take the illustration shown here, for example percent of Americans are currently considered healthy. However, 12.3 percent have diabetes. The majority of people on the escalator are not aware they are headed that way. If you have any components of metabolic syndrome, you may be moving toward serious disease. Personal Health Assessment— All About You
Helps you identify your current health status and identify potential risks or issues 35 questions Questions change depending on your answers Takes just 15 minutes Results stored online in a secure personal account Let’s take a few minutes to talk through each of these components. The first step in participating in Designed Wellness is taking your Personal Health Assessment. It’s an online survey designed to help you identify your current health status and any potential issues or risks so that you can put a plan in place to address them. Think of it as a snapshot of your current health! The survey consists of 35 questions, which change depending on how you respond. It takes just 15 minutes to complete, and your results are stored online in a secure personal account for you to review in the future.

Biometric Screening—Greater Insight
Cholesterol Blood glucose (sugar) levels Triglycerides Blood pressure Body mass index (BMI) 15 min Takes approximately minutes (On-site screening) The second step in participating in Designed Wellness is completing a free biometric screening. It’s a full biometric panel consisting of, but not limited to, measuring your cholesterol and blood glucose (or sugar), measuring your triglycerides levels via a finger-stick blood draw, taking your blood pressure and calculating your body mass index (or BMI). Your screening will take approximately 10 to 15 minutes.

Three ways to participate: 1 2 3 On-site screening With your personal physician Via LabCorp our Community Access Partner You will have three ways to participate in the biometric screening: First, you can get a screening on site—more information is coming soon. Second, you can get a screening with your personal physician and have your physician complete and submit a form. Third, you can get a screening with our Community Access Partner, LabCorp. Next Steps Screening Results*
2 or fewer Risk Factors Earn an additional $150 by completing the Healthy Activities of your choice Earn $150 Screening Results* Complete Health Improvement Program, Earn $150 3 or more Risk Factors Your biometric screening results will determine your next steps. If it is found that you have three or more Risk Factors for metabolic syndrome, or your fasting glucose score exceeds 100, you must complete at least eight weeks of a Health Improvement Program. However, if you have two or fewer Risk Factors, you will not be required to complete a Health Improvement Program, and you will automatically earn a $150 incentive. In either case, you will have the opportunity to earn an additional $150 by completing the Healthy Activities of your choice. More on those later. *Your results will be completely confidential and uploaded to the secure web portal

Health Improvement Program Options
As I mentioned before, if it is found that you have three or more Risk Factors for metabolic syndrome, or your fasting glucose score exceeds 100, you must complete at least eight weeks of a Health Improvement Program to satisfy the Designed Wellness requirement. The good news is you have the choice of participating in one of two nationally recognized, state-of-the-art programs—at no cost to you!

Naturally Slim focuses primarily on weight loss and metabolic syndrome reduction. You work on changing how you eat instead of what you eat. On the other hand, Real Appeal is more focused on changing your lifestyle using scientific principles. Both programs help you take small, steady, sustainable steps to building a healthier future. Both programs are online and offer coaching, and each comes with its own set of tools, including a robust member website that allows you to track your progress and get support from the community.

Health Improvement Program Options
Naturally Slim Real Appeal Full Program Duration Up to 10 weeks Up to 52 weeks AHIP Requirement 8 weeks You can participate in the Naturally Slim program for up to 10 weeks, and in the Real Appeal program for up to 52 weeks. However, the minimum requirement for Designed Wellness is 8 weeks’ participation. However, rest easy: The programs are not outcome-based. Rather, they are participation-based. Even though you may not reach your goals within the 8 weeks, you’ll still meet the Designed Wellness requirement by participating!
