Located on the right-hand side of the CR&A Home Page, the new Claim/Leave Search tool is a great way to quickly look up the claim and/or leave information of an individual claimant. An individual’s claim/leave history can be retrieved simply by searching by his/her name, Social Security Number (SSN), the claim number or the leave number. When searching for an individual claimant, this powerful tool is a great substitute to running a report.

Start by entering either the claimant’s last name, SSN, claim number or leave number.
Hint: If you are searching by the claimant’s last name, also adding the first name (if known) will result in a smaller list of names. Hint 2: In order to protect the privacy of all our customers, we encourage you to utilize the lowest SSN display level possible. When the search results appear, simply click on the name you want. Note: A claimant’s name will appear more than once (in the search results) if there are multiple records in which the Employee ID (EEID) appears on one or more records, and doesn’t appear on others. Clicking on either name will provide the same results.

Here are the final search results. A couple of points to note:
Clicking on a claim or leave number will provide additional information. When disability status and leave status information share the same row in the search results, there is an association between the two events. For example, in the results above, the first SI-STD claim listed ( ) has an associated FMLA leave ( ). However, the second and third claims do not.
