3 interRAI assessments used in New Zealand
In the community: Contact Assessment (CA), a basic screening assessment for people living in the community Community Health Assessment (CHA) and Home Care Assessment (HC) for planning care and services in home and community-based settings Palliative Care Assessment (PC) for community-based older adults where a palliative care focus is required. In aged residential care: Long Term Care Facilities Assessment (LTCF) for evaluating the needs, strengths and preferences of those in aged residential care.

5 What interRAI tells us about people with dementia living in the community
All the data included in this presentation is 2017/18 HC assessments unless specify otherwise. Assessment count = 37176

6 What we are looking at Home Care assessments completed in 2017/18 Assessments not clients Demographic characteristics Outcome measures Outcome scales Clinical Assessment Protocols (CAPs) Data presented here are from Home Care assessments completed between July 2017 to June (n = 37,176) Counts and percentage of assessment rather than clients – because one person could have multiple assessments in a year which affects the results. Automatically generated using built in algorithms in the assessment Outcome measures are usually used to evaluate an older person’s current clinical and functional status. A triggered CAP is usually an opportunity to facilitate improvement, slow rate of decline or alleviate symptoms.

One in four Home Care clients had a diagnosis of Alzheimer’s disease and/or other type of dementia 44% of diagnosed Home Care clients are between 75 to 84 years of age Men were 12.5% more likely to be diagnosed with dementia (age adjusted) Māori (31%) and Pacific people (35%) are more likely to be diagnosed with dementia (age adjusted)

8 Cognitive performance Scales
This scale combines information on: daily decision making short term memory making self understood eating performance. The score ranges from 0 to 6 – the higher the score, the worse the cognitive impairment.

13 Family carers status This measure captures the feelings of distress, anger or depression of the primary family carer. The measure is only applicable to assessments completed in the home and the community only. It is important to know how well the family carers are doing, their well-being have direct impact on the people they are caring for. From interRAI Home care assessment, 45% of the family carer of people living at home with dementia have reported feelings of distress, anger or depression. This percentage is about a third for Home care client without dementia or cognitive loss.
