Why are we here? To update you on what we have done since the last time we met To let you know how our plans are coming along Ask for your views on our future plans Start well, live well, age well

As a reminder… We visited Share Community on 25th July 2016 We spoke about our South West London Transforming Care Partnerships (TCP) plan The plan explains how we can improve services for people with learning disabilities and/or autism – especially those living in special hospitals. We also asked your advice on where we can speak to people about this plan Start well, live well, age well

Who have we spoken to? You told us that we needed to speak to people in Wandsworth with learning disabilities and/or autism, their carers and families We visited Share Community (as part of the grassroots events) Hope Community Wandsworth Learning Disability Alliance We listened to people’s stories and they helped us to improve our plan. Since then we have… Helped people who have been staying in special hospitals to come back and live in the community Looked at ways to stop people going to special hospital Worked with our colleagues to make sure people get the help that they need in the community Improved how we talk to each other to help us plan for people’s needs Start well, live well, age well

Since then we have… Looked at what services need to be available in community that aren’t already there. Shared more information about Personal Health Budgets Organised a project to talk to more people who have stayed in a special hospital about our ideas Start well, live well, age well

8 What we will be doing next?
Helping more people who live in special hospitals to live in the community. Especially those who have been living in special hospitals a long time. Making support better for those who are in distress or have challenging behaviour (Crisis Support) Making a plan to help us make sure we have the right staff in place and more houses to support people to live in the community Talking to people who have been in special hospitals to listen to their experience to make our plans better Start well, live well, age well

What do you think is most important to people who are distressed (in a crisis)? Being treated close to where they live? Being treated in their own home? Being treated in a special home? Anything else? Thinking about the staff that support people when they are distressed: What skills do you think they should have? How should they speak to people? Other thoughts?
