We visited Share Community on 25th July 2016
We spoke about our South West London Transforming Care Partnerships (TCP) plan
The plan explains how we can improve services for people with learning disabilities and/or autism – especially those living in special hospitals.
We also asked your advice on where we can speak to people about this plan

Since then we have…
Helped people who have been staying in special hospitals to come back and live in the community
Looked at ways to stop people going to special hospital
Worked with our colleagues to make sure people get the help that they need in the community
Improved how we talk to each other to help us plan for people’s needs

Looked at what services need to be available in community that aren’t already there.
Shared more information about Personal Health Budgets
Organised a project to talk to more people who have stayed in a special hospital about our ideas

8. What we will be doing next?
Helping more people who live in special hospitals to live in the community. Especially those who have been living in special hospitals a long time.
Making support better for those who are in distress or have challenging behaviour (Crisis Support)
Making a plan to help us make sure we have the right staff in place and more houses to support people to live in the community
Talking to people who have been in special hospitals to listen to their experience to make our plans better
