CLINICIAN INFORMATION Thank you for your assistance in this British Association of Head and Neck Oncologists (BAHNO) multi-centre audit of head and neck cancer, post-treatment surveillance. This audit is being delivered by INTEGRATE, the National ENT Trainee Research Network. The objective is to examine adherence to accepted national standards of care. This will help us to understand current practices, including identifying gaps and areas of need, to facilitate improvements and stimulate relevant research in the future. During the audit window we kindly request that clinicians complete clinic audit proformas for all consultations that meet the inclusion criteria. This can be performed in place of or alongside normal clinical record keeping, dependent on local procedures. Further data will be extracted from the patients’ notes to enable assessment of all audit standards. All patient data will be anonymous and clinicians and sites unidentifiable in the subsequent analysis and reporting. INCLUSION/EXCLUSION CRITERIA ALL outpatient consultations for the surveillance of post-treatment head & neck cancer patients. With the exclusion of thyroid and cutaneous malignancy. PATIENT INFORMATION The British Association of Head and Neck Oncologists (BAHNO) are conducting an audit of head and neck cancer surveillance at this centre. This audit is being delivered by INTEGRATE, the National ENT Trainee Research Network. The BAHNO Head & Neck Cancer Surveillance Audit 2018 is collecting information regarding the follow up and treatment of head and neck cancer patients throughout the UK. Information will be collected anonymously, so individual patients will not be identifiable, and will help us to improve the way we treat Head and Neck Cancer patients in the future. The hospital providing your care will safely send the information to the audit database where it will be securely stored. We ensure the data collected is subject to strict rules of confidentiality as laid down by acts of parliament, including the Data Protection Act 1998 and the Health and Social Care Act 2001. Large audits are most effective when they have information from as many patients as possible. However, if you do not wish your information to be used please tell the people who are treating you. Declining to be part of this audit will not affect your treatment in any way.
