Table A has been designed to:
The Monthly Rain Activity Report consists of a form header section and two tables. Table A has been designed to: Calendar the month’s rain events Provide real-time documentation as to if off-site discharge occurred, and Provide a reliable reference source to use to complete other monthly reporting requirements. click

The data recorded in Tables A and B must support each other.
Table B consists of two sections. Section 1 is the No Discharge Inspection Log and has been designed to provide a daily recording of any and all rain events that occurred during business hours but did not produce off-site storm water discharge. Section 2 is the No Discharge During Business Hours Log. The purpose here is to document rain events that occur during business hours that results in run-off/discharge either before or after normal business hours. The data recorded in Tables A and B must support each other. click

2 3 4 5 6 1: Record facility name as it is registered with ECMS. 2: Record the facility’s WDID number. 3 & 4: Record the name and title of person conducting the inspection. 5: Record the MONTH and YEAR the inspection was done. 6: Indicate which program you are a member by checking the appropriate box. click

1 2 3 1 2 3 1: You do not record anything in the Date box (box 1). The numbers in this box correspond to the dates of the month. This report is to be completed DAILY. 2 For each date, indicate if it rained or not by checking the appropriate box. 3: For each date, indicate if off-site discharge was occurred during normal business hours. For any date where it rained during normal business hours, but no off-site storm water discharge/run-off occurred, you will need to record that date on Table B of the Monthly Rain Activity Report click

4 5 #4 & #5: Enter the name and title of the PPT member who conducted the inspection. 1 #1: Enter the DATE it rained. All dates are to be formatted as dd/mm/yyyy 3 #3: Enter the time you inspected the discharge locations when you determined there was no off-site discharge. Remember that the time you record needs to be during normal business hours. 2 #2: Enter the date you inspected the discharge locations when you determined there was no off-site discharge to report. click
