2. How to Start
Touch on the HEALTH application icon After a few second login screen will appear on your mobile.

3. GPS Location must be Required

4. How to get IMEI number from Application
.First Time Internet Connection is Mandatory
Once the Internet connection is received, it will automatically go to the login page.
You Can Check your IMEI Number on Login Page

5. LOGIN Screen
In the users name and password box, type the user name and password provided and then touch the login button. Based on the mobile IMEI number, the user Name and password are matched by the application. If the user Name and password are found correct then they are logged into the application and the home screen of the application becomes available on your mobile.

6. First time Internet Connection Required
Plese turn on Internet connection
for the first time !!

7. Login After Click on Login Button Verifying Process will be start

10. New Entry After Download Health Facility User Can Input Their
Data By Clicking On New
Entry Button.

11. Facility Reference Select Facility Reference Number.
In Entry Page First User have to select the facility Reference Number.
Once we Click on Drop Down
The following option will apear.

12. Health Facility After Filling All the needed Data We have to capture
Health Center Photo by Clicking on Take Photo
Image.
Click here

13. Take Geo tag Photograph
We have to wait for
G.P.S .
Once G.P.S
Will be stable
Take Photo Button will be
Appear.

14. Take GPS location
After taking picture We have to click on Save Button .
All Data will be save on the phone (Local)
Click for Save

15. Uploading GPS location
For Sending data on server
We have to click on UPLOAD
Button.
After Click on the upload
button A pop will be open
For YES or NO.
Click on yes data will be upload
To the server.
