Frank had an abnormal cancer screening test. He came to the clinic for a stereotactic biopsy. The nurse (Ms. Abby) gave Frank a local anesthesia to reduce the discomfort of the biopsy procedure. We need 2D6 to change codeine into morphine. There needs to be less 2D6 than 3A4 for your liver/body to be able to metabolize or break down codeine into morphine. Morphine is why most people experience pain relief. Frank: So why not me? With too much, you would be like that other patient. Too much grapefruit juice can reduce your 3A4 so that you have way too much 2D6 … and then you metabolize the medication too fast for your body to benefit from pain relief.
