3 Evacuation Procedures
If evacuation is ordered, follow these procedures 1. Stay calm, do not rush, and do not panic 2. Safely stop your work. 3. If you have prescription medications and it’s conveniently located by you, you may bring it with you. Do not waste time looking for them. 4. Proceed to the nearest exit (most exits are located by signs, please familiarize yourself with your environment) 5. Proceed to designated assembly area (assembly area A-K) and report to your manager and await further instructions.

4 Alarm activation and notification
To activate the alarm locate a pull station and pull lever, there is one in every building (refer to evacuation map) When alarm goes off, you will hear a LOUD continuous beep

9 First Aid/AED Basic First Aid (i.e. Band Aids), First Aid Kits are located all buildings (refer to evacuation maps) AED’s are located all buildings (refer to evacuation maps)

12 Chemicals Chemicals must be approved prior to entering the Sunnyvale Facility by EHS A safety data sheet must be provided If approved, a copy of the safety data sheet will be placed on Teradyne’s online chemical management database “ACTIO” Actio is available through the internal EHS website In the event the system is unavailable contact EHS. Backup CDs are available.

The type of chemicals most often used within the Sunnyvale facility include: Loctite Flux Isopropyl Alcohol Tin/Lead Solder

14 Hazard Assessments Hazard Assessments have been conducted to determine appropriate PPE for a job/area. PPE Protects “YOU”! Your supervisor/manager is responsible for informing you of the required PPE for your work area PPE is based on the job activity An example of the hazard assessment is on the following slide. The assessment lists the location, job description, hazard description, safe work conditions and appropriate PPE

16 Electrical Safe Work Practices
Electrical Safe Work Practice Areas are those areas within the Litepoint facility that require additional knowledge of the activities and electrical hazards in the area to work. While it is every employees responsibility to work and act in a safe manner, there are certain locations in the Litepoint facility that require additional Electrical work practice awareness and procedures. If you work in these areas, you will be required to complete additional training:

17 Teradyne Environmental Commitments
Environmental initiatives require Teradyne and its division (Litepoint) to reduce the use of fuel, electricity, water, hazardous materials and so forth. These initiatives include: ISO (Litepoint site is not certified) Carbon Disclosure Project All programs require reporting, tracking, and your cooperation.

Understand the impact of the activities you perform daily What type of wastes do you generate Do you use or deplete natural resources Do you use hazardous or regulated substances Learn what you can do to reduce the impact of your activities on the environment Recycle Conserve energy and water Switch to less hazardous substances
