Community Strengths and Themes Assessment
Provides qualitative information on how communities perceive their health and quality of life concerns as well as their knowledge of community resources and assets.

Local Public Health System Assessment
Measures the capacity and performance of the local public health system by surveying all organizations and entities that contribute to the public's health. …… It’s complicated…..

Forces of Change Assessment

Health Status Report Provides quantitative data on a broad array of health indicators, including quality of life, behavioral risk factors, and other measures that reflect a broad definition of health.

Community Asset List In each county, there are a number of resources that can be used to help counter some of our most pressing health concerns. This list was compiled with issues from the Community Health Assessments in mind, and will be updated annually.
