In the Birmingham 3-step for stroke prevention1, step 1 is to identify low-risk patients (CHA2DS2-VASc score of 0 in men or 1 in women, with event rates of <1% per year) who do not require antithrombotic therapy. Step 2 is to offer stroke prevention to those with one or more risk factors for stroke. The SAMe-TT2R2 score can help to decide between a vitamin K antagonist (VKA) with good time in therapeutic range (TTR) or a non-VKA oral anticoagulant (NOAC; step 3). OAC, oral anticoagulation.
