6 Objectives Upon completion of this lesson, learners will be able to:
Name the types of services that are provided by hospice Describe the purpose and provisions of a service agreement in hospice Describe how patient emergencies or discharges are handled in hospice TOC

7 Why is This Relevant? TOC
Learning about the hospice services, service agreements, emergencies and discontinuing services will assist staff in supporting and advocating for patients during their end of life care TOC

8 Licensure Provisions Hospice patient: Hospice service means: TOC
A hospice eligible person has terminal illness with less than one year to live Hospice service means: Palliative and supportive care to meet all needs of patient and family in final stages of illness, dying and bereavement directed by Inter-Disciplinary Group (or IDT) TOC

9 Licensure Provisions Hospice Programs must ensure at least two core services listed here are regularly provided: Physician Registered Nurse Medical Social Worker Counseling Services TOC

10 Licensure Provisions Hospice Programs must also provide, if needed:
Physical Therapy Occupational Therapy Speech Therapy Home Health (hospice) Aide Volunteers These services must be available 24 hours a day: Physician Nursing Pharmacy If necessary, inpatient hospital care for pain control or symptom management, respite care may provided in SNF or hospital TOC

11 Plan of Care An interdisciplinary team must complete
an individualized and comprehensive assessment of needs for each hospice patient and family FHCH hospice must provide the hospice patient or responsible person a copy of the initial plan of care and changes upon request The interdisciplinary team coordinates care with any persons or entity involved in care to ensure that services in the plan of care are provided TOC

12 Service Agreements The Service Agreement describes the services
Hospice must provide written list of charges within 48 hours of admission The Service Agreement describes the services A copy is left in the patient’s home chart Hospice must document and communicate contingency plans Note: Categories and disciplines refer to physical therapy Speech therapy, nursing, Note: HHA or HMK will be included In the Nursing or therapy Service Agreement TOC

13 Service Agreements Contingency Plans:
The action to be taken by the hospice, patient, or responsible person if scheduled services cannot be provided The method for a patient to contact a responsible person whenever staff are providing services The method for hospice provider to contact hospice patient’s responsible person (if a responsible person is named) TOC

14 Handling Emergencies Hospice staff must complete an orientation and be trained in the handling of emergencies and use of emergency services The hospice provider must ensure the patient’s record contains a copy of the patient’s health care directive, if executed and available TOC

15 Complaint Procedure Hospice is required to: Have a complaint procedure
Provide patients with the telephone numbers of these programs: The Office of Health Facilities Complaints The Office of Ombudsman for Older Minnesotans Review any patient care complaints by the interdisciplinary team. Take remedial action as appropriate TOC

16 Discontinuing Hospice
If the hospice discharges or transfers a patient for any reason, the reason must be documented in the clinical record including: The reason for discharge or transfer Why the patient’s needs cannot be met by the hospice agency, if hospice services are still needed The hospice must also provide the patient or responsible party with a written list of providers that provide similar services in the patient’s geographical area TOC

17 Summary Hospice licensure law:
Describes the unique rules and regulations for the protection of patients throughout their care Informs staff of the processes used to address patient concerns, complaints or emergencies Describes how staff can ensure patients’ decisions are known, communicated and respected TOC
