1. Serve meals to all needy children 18 and under (or persons 19 or older with a public or private nonprofit school program for the mentally or physically disabled.) 2. Serve meals that meet the minimum meal pattern requirements. 3. Abide by all safety measures as stated in the food safety regulations.

4. Provide adequate supervision during the meal service. 5. Maintain and submit promptly such reports that the sponsor requires. a. Temperature logs b. Accurate participation counts 6. Report to the sponsor any changes in the number of meals required as attendance varies.

Notes Call school cafeteria with counts needed for the day.
Call by 8:00 am to :________________ Pick up meals on time each day. Milk must be in cooler or under ice. Count sheets from previous day should be completed and turned in before meals can be released for current day.

Notes Give meal and milk directly to children. (This is a reimbursable meal and all components are packed). 1 breakfast at breakfast + 1 milk 1 lunch at lunch + 1 milk Do not give to adult for child or children. As long as you have meals, no child should be turned away. Children can take only one meal. One breakfast at breakfast. If child wants a 2nd and it is available, count as another breakfast. One lunch at lunch. If a second meal is desired, count as adult lunch. $3.50 for lunch.

Notes Adult Meals One adult meal is allowed for each meal service period. (Document on Meal Count Sheet). If other adults want meals they can have for a charge of $2.00 for breakfast and $3.50 for lunch. (Document this on the meal count sheets). Give money to school cafeteria. Meal Count Sheets - Check number from list after each meal is served. Do not allow more than one child to have a meal without checking list. Keep accurate count. Must be completed before meals are received next day.

Must be in reading distance for all children. (If from a van, make sure it is posted in windshield). (If in a room, make sure it is posted on a wall or window). (If on a cart, tape to the cart). Store leftovers for next day in cooler. Leftovers from previous day should be used first for the following day. Leftovers must be kept under refrigeration overnight. Call school kitchen to report number of meals needed for the day. Call by 8:00 am to: ____________________.
