Why are we here? To give you an update since our last visit To let you know how our plans are coming along Ask for your views on our future plans Start well, live well, age well

4 A reminder of what we spoke about..
We visited Richmond Working Together on 10th May 2016 We spoke about our South West London Transforming Care Partnerships (TCP) plan The plan explains how we can improve services for people with learning disabilities and/or autism. We also asked your advice on where we can speak to people about this plan Start well, live well, age well

Who have we spoken to? You told us that we needed to speak to people in Richmond with learning disabilities and/or autism, their carers and families We visited Whitton True Access Octagon Club Transition Club SEND Family voices We listened to people’s stories. This helped us to improve our plan. Start well, live well, age well

Since then we have… Helped people who have been staying in special hospitals to come back and live in the community Looked at ways to stop people going to special hospital Worked with our colleagues to make sure people get the help that they need in the community Improved how we talk to each other to plan for people’s needs Start well, live well, age well

Since then we have… Looked at what services need to be available in community care that aren’t there: We will be looking at crisis care and how to better train staff (positive behavioural support) Shared more information about Personal Health Budgets Organised a project to talk to more people who have stayed in a special hospital about our ideas Start well, live well, age well

8 What we will be doing next?
Help more people who live in special hospitals to live in the community. Especially those who have been living in special hospitals a long time. Make crisis support services better Make a plan to help us make sure we have the right staff in place and more houses to support people to live in the community Talk to people who have been in special hospitals to listen to their experience to make our plans better Start well, live well, age well
