CNA Assess & Identify needs by using quantitative and qualitative data at FAC; assess resources and agency IS data; Determine top 5 – 8 needs to target – FAC; Establish systematic process to determine priorities – (root causes, cause and effect, force field analysis, etc.)

Strategic Plan 3 – 5 years into the future;
Determine the plan for the SP; Reevaluate mission statement; Determine vision statement; Clarify shared values; Roll top 5 – 8 needs from CNA into SP, FAC (Assessment); SWOT Analysis – SO Strategies; ST Strategies; WO Strategies; WT Strategies

Establish quantitative and/or qualitative goals based on analysis of data (Planning); Set SMART objectives (Planning); Establish strategies (Planning); Determine date to start strategies/services (Planning);NPIs, org standards, ROMA module Start strategies/services (Implementation); Monitor, conduct follow up and reporting (Achievement of Results)

Identify procedures by which results data will be analyzed; Conduct analysis of results data for information and reassessment(Evaluation).

Community Action Plan Annual work plan derived from CNA and strategic plan (Assessment); Service Delivery System/Case Management System; Partnerships/linkages; Resources; Outcome Measures, NPIs, other, org standards, ROMA module (Planning); Strategies (Planning and Implementation)

Community Action Plan monitoring, follow-up and reporting (Achievement of Results); Analysis of Results, reassessment (Evaluation)

summation The key to successful achievement of short term, intermediate and long term goals is assessment of needs; planning using assessment data, agency data and resources; determining what services will meet identified needs using a theory of change; successfully implementing services; tracking progress, documenting and reporting results; and, conducting analysis of outcome data for evaluation and reassessment purposes.
