HBA1C BPS/BPD BMI Find comparative data for your population Set goals Use built-in automated reports when possible Implement custom reports if necessary Document how to run and use the reports

Extract / transform / load
Volunteer Reports (Volgistics) EHR Reports (MedInformatix) 3rd-party product (Reporter) Extracts data from EHR (MedInformatix) Applies constraints, such as Diabetes diagnosis Exports data to Excel spreadsheet

Check, clean, prepare Review key Excel columns for missing or incorrect data, for example: Replace BMI values with BMI formula Add missing height data Delete rows with missing weight data Ask the question, “Does the data make sense?” If not, remove it. Document changes to data

Analyze and interpret Calculate key performance indicators using Excel functions Ask the question, “Do the calculated results make sense?” If not, go back to the previous step. Record results in dashboards
