Pathways is a short term rehabilitation program designed to bridge the gap between hospital and home for individuals recovering for illness or injury.

5. FINANCIAL MATTERS
Our team guides the guest and their family through the admission process…As part of our services we research Medicare and Insurance and take care of all billing procedures

6. PHYSICAL, OCCUPATIONAL, & SPEECH THERAPIES
Obviously this is a key component of the program. A home evaluation may be included prior to discharge.

7. NURSING CARE & EDUCATION
Our nursing staff will work with guests and their families to educate them on management of their health care at home. Nursing staff also works closely with therapy to ensure guests are walking and using adaptive
equipment as directed by therapy…

8. DIETARY SERVICES
Overseen by a licensed dietician – guests’ diets are monitored to promote optimal healing following a hospital stay. Dietary staff can also provide
education to guests and families
regarding dietary restrictions and
needs.

9. SOCIAL SERVICES
Upon discharge many individual will require continued care and services at home. Social services educates guests and families on available resources and assists in obtaining needed medical equipment and services.
Discharge planning is a VERY important
Component of the program.

10. PATHWAYS AS A PLACE
Facilities with a separate Medicare wing can take the Pathways program to the next level. Because our guests do not want to come to a “Nursing Home” they will be especially excited to hear that they can come to a special wing surrounded by others whom like themselves are working toward the goal to return home.
The Unit should be renamed Pathways and referred to as such by all staff. This distinction should be made when marketing to physicians, discharge planners, etc…
