We have 5 locations thought out the state with two Military Service Centers Serving All Branches of Military

2 What do we do? The purpose of the Family Assistance Centers (FACs) is to provide support and assistance to Service Members and their Families by offering information about military, federal, state, and local resources. FACs provide a variety of services to all branches of the military, our services include: ID Cards and DEERS Enrollment TRICARE Benefits -Provide information on DEERS Rapids Centers -Provide general information on insurance benefits -Assist with obtaining ID cards -Assist with TRICARE medical and Dental Applications -Assist with finding local participating providers Financial -Assist with Challenging claim denials -Understanding Military pay -Assisting with Financial assistance and loan applications Crisis Intervention and Referral through the American Red Cross and other agencies -Domestic Abuse -Referral to availability of outside grants -Child Abuse/Neglect -Provide referrals for budgeting classes -Danger to self and/or others -Abuse/Neglect of a Vulnerable Adult Community Resources Provide a list of local community resources including but not limited to: Legal -Assist with Child Care Resources and Referral -Assistance with Power of Attorney/Wills -Assist with referral to local financial resources -Information and assistance with Service Member Civil Relief Act -Assist with finding a local FRG group -Assistance with referral for Employer Support to the Guard and Reserve(ESGR) -Assist with finding any community specific services(Food Pantry, car repair, etc.)

Provide information on: ID card eligibility (Must have two forms of ID) How to update and verify DEERS data Locating DEERS RAPIDS centers Assist with establishing DEERS appointments if needed

5 Tricare Resources Information & Referral
Health benefits and programs Medical and dental applications Local participating providers

6 Crisis Intervention & Referral
Five types of Crisis Situations: Domestic Abuse Child Abuse and Neglect Danger to Self Danger to Others Abuse or Neglect of a Vulnerable Adult

7 Community Resource Information & Referral
Supplementary service to geographically dispersed families Local, regional and national community programs. Examples: Child care resources: Education grants and benefits

Network with local individuals, programs organizations, agencies and businesses. Finding the resources for military families with community forces and/or Inter Service Family Assistance Committee (ISFAC) groups.

11 Additional Services For Families of Deployed Service Members
Distribution of information as directed by the Unit Commander, Family Readiness Support Assistance (FRSA), or leadership within the Family Program Office. Monthly wellness calls to primary points of contacts.

12 Who do we contact? Each Service Member fills out a Family Data Questionnaire. This form allows the Service Member to identify individual(s) who would like to receive information. The Service Member is the only one who can add a family member. If you or someone you know would like to be added to the contact list, please contact your Service Member and ask them to make the request. The names and information we obtain is confidential and will not be shared without the approval of the Service Member.

13 Ways we contact Family Assistance Centers (FACs) incorporate different types of communication styles to reach family members with information and resources pertaining to military, federal, state, and local resources. Telephone Mail In person Blackboard Blackboard is an automated phone system used to distribute critical information in the course of events of a Unit deployment and notification of significant events concerning the Unit or Family Readiness Group (FRG) that cannot be accomplished through due to time constraints.

14 Wellness Calls A FAC Specialist will contact the Service Member’s primary point of contact every 30 days. The purpose of these calls and s is…. See if there are any needs or resources we can assist with Share information from the Unit Commander or FRG group Answer any questions Ensure the contact information provided by the Service Member is still accurate. As FACS we are required to make contact with the Primary Contact every month. If we call you or you please respond back so we can assist you with whatever your needs may be. We don’t want to harass you if you don’t respond to our initial contact but we do have to try more than one way to get a hold of your. The primary goal is to make sure you are doing ok and have the assistance and resources you and your family need during the deployment. Please keep us in mind if you are moving – we would appreciate receiving your updated information!
