Title of Subject: Auxiliary Face Fan Last Reviewed Date: November 2015 SAFE WORK INSTRUCTIONS The following safe work instructions in NOT INTENDED to replace hands on training requirements of the code of federal regulations. The intent is to provide an overview of known potential safety hazards associated with equipment operation. Hands on training with an experienced operator on each specific model must be conducted. The trainee can not proceed onto production until they are determined ready to proceed by the supervisor of the training. Before any operation or maintenance procedure is performed with or on this equipment, it is important that personnel have a complete understanding of the location and functions of all controls and indicators, as well as, the location and use of all emergency shutdown controls and procedures. Steps for Initial SWI Training: Prepare – Review the SWI. Present – Quick Card at the machine with experienced operator. Tryout – Person performs the task with supervision. Follow up – Competency points are verified by supervisor and is signed by the trainee/trainer to complete the training. Title: Auxiliary Face Fan SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Pre-ops: Examine the workplace for loose materials in the roof and ribs. Examine the walkways for potential slipping/tripping hazards. Check the cable plug, receptacle, breaker and restraining clamp at the power center. Follow the cable from the power center to the equipment and check for cuts, worn or bare places and number of splices. Maintain equipment free from accumulations of combustible materials. (Coal, dust, paper, grease and oil). Test all levers and switches to ensure proper operation. Test all emergency stops and panic switches to ensure proper operation. Ensure that the fire suppression system is intact and all required items are present. If equipment is located at or inby the last open crosscut a methane examination must be performed prior to energizing the equipment. Ensure that the screens are in place at both ends of the fan and not damaged. Sound the audible alarm prior to starting for any of the checks. Scale loose roof and ribs as necessary. Remove potential slipping/tripping hazards prior to beginning task. Cable plug, receptacle and breaker must have matching ID tags. Restraining clamp must be secured. When possible the power will be removed during this check. Do not handle damaged cable while energized. Levers and switches must not stick and must center when released. All electrical equipment must be equipped with a fire suppression system or have a fire extinguisher. Remove any debris or accumulations detected at the intake end of the fan. Hearing protection is required to be worn when working near or inby the fan location. Ensure that all personnel are in the clear prior to starting the fan for any of the checks.

Title: Auxiliary Face Fan SAFE WORK INSTRUCTIONS KEY POINTS SAFETY PRECAUTIONS Tramming: Prior to tramming the fan examine the tram path for loose materials in the roof or ribs. Examine the tram path for potential slipping/tripping hazards. Sound the audible alarm prior to starting or tramming to alert others of your intended movement. Utilize proper body positioning during tramming. Utilize effective communications between co-workers. (Radios and cap light signals , voice) Ensure that the pump motor only is running during the tramming of the fan. Scale loose roof and ribs as necessary. Remove potential slipping/tripping hazards prior to beginning task. Remove or hang cables, waterline prior to tramming. Ensure that all personnel are in the clear prior to starting or tramming. Do not tram past or allow anyone to stand next to the fan while it is tramming. Uneven or slick bottom conditions can cause unexpected movement. Always inform anyone in the area of your intended movement. Fan is equipped so that tram cannot be enabled with the fan running. Fan Duster: Prior to putting dust in the hopper examine the roof and ribs for loose materials. Examine the area around the hopper for potential slipping/tripping hazards. Ensure that the power to the duster is removed prior to removing lid to fill. Ensure that caps are installed and secured on the hopper prior to energizing the duster. Utilize proper body positioning and safe lifting techniques. Hearing protection is required when working near or inby the fan. Hooking/Unhooking Ventilation Devices: A minimum of two people are required when hanging or removing tubes.
