ELIGIBILITY
Eligible employees: All full-time and part-time regular employees All hired prior to 1 May 2018 can enroll in the plan not later than 1 August All hired on/or after 1 May 2018 can enroll any time after joining Concentrix.
Eligible dependents: Spouse and partners (18-64 years); children (2-26 years); parents up to 64 years Dependents of current eligible employees can be enrolled no later than 1 June Newly hired eligible employees should enroll their dependents when joining the plan.

Out-of-hospital medical aid up to BGN2,000 per one insured per full insurance year
Hospital medical aid up to BGN10,000 per one insured per full insurance year
Dental Care up to BGN 150 per year (w/o co-financing)
Reimbursement of expenses for medicines and auxiliary means up to BGN 150 per year (w/o co-financing)

All enrollments received on/or after 25 April 2018 will be considered for joining as of 1 June 2018
Forms are available for download under Bulgaria HR Admin/Employee Benefits category.
Each enrolled employee must personally collect the Generali card when he/she will physically sign the authority/authorization form.
