27/May Haima to Qarn Alam black top-road
Two pedestrians wanted to cross the road. One stopped and looked left and right. The other stepped onto the road without looking and was hit by a passing Landcruiser He died on the spot. 18/June/2000

Discuss in your team (27/May fatality): If you were the pedestrian that died: - How could you have prevented this accident ? - Why, do you think, did he not do these things ? If you were his colleague (the one that stopped): - How could you have prevented this accident ? - Should you warn a colleague of an obvious Hazard ? If you were the driver of the landcruiser: - How could you have prevented this accident ? - Would a warning signal with the horn have helped ? - Would you have thought that the man would cross without looking ? Would you have been prepared ? Pedestrian: How could you have prevented this accident ? By stopping, looking left and right and waiting until the landcruiser had past Why, do you think, did he not do these things ? Could have been tired Could have his mind on other things: family, financial problems, etc. He did stop, looked right and crossed the road. He did not look left. He had come from Pakistan just 2 weeks ago. In Pakistan driving is on the left!!! So a pedestrian looks right first and only then to the left!! Colleague: How could you have prevented this accident ? Watch you buddy and make him aware of the road and oncoming landcruiser (even if it is very obvious). Hold him back with your arm. Should you warn a colleague of an obvious Hazard ? Yes you should. He might not see the danger, he might be less experienced. He might not be concentrating. Driver: How could you have prevented this accident ? Warn the pedestrians with a blast of the horn Slow down (expecting the pedestrians to cross the road) Would a warning signal with the horn have helped ? Probably YES Would you have thought that the man would cross without looking ? Yes/No/Maybe ! Would you have been prepared ? Yes/No/Maybe. You should always be prepared! 18/June/2000

Canter driver: How could you have prevented this accident ? Concentrate on the driving, stay on the right side of the road. Why, do you think, did he not do these things ? Chatting and joking with the passenger. Worries about family, work, or other Tired Passenger: Not distracting the driver by joking with him (normal conversation is OK) Warn the driver you notice he is on the wrong side of the road (buddy system). Should you warn a driver of an obvious Hazard ? Yes, unless it is clear from the drivers behavior/action that he is already aware of it Driver of Truck: How could you have prevented this accident ? Give a horn signal instead of flashing lights (or in addition). Lights are not so visible in the strong sun light. Slow down if still possible. Pull off the road even more. Would a warning signal with the horn have helped ? Probably yes but there is no guarantee Would you have thought that the canter would come on your side of the road? Yes/No/Maybe. Suddenly the rear closure slammed open with enormous force
Suddenly the rear closure slammed open with enormous force. The helper, who stood behind it, was thrown 13 m away. He died on the spot. It is not yet clear why the closure/door failed: Faulty Relief Valve? - Faulty pump pressure switch? - Internal explosion? (no earthing was used) - Bolt failure (8x 2”dia)? Press Gauge 18/June/2000

If you were truck driver: - How could you have prevented this accident ? - Would you have “seen” the Hazard ? - Should you warn a colleague of an obvious Hazard ? If you were the supervisor/manager of this team: - How could you have prevented this accident ? - Do you have procedures for these type operations ? - Does your staff know + follow these procedures ? - How do you ensure all Hazards are identified + controlled ? Helper: How could you have prevented this accident ? By not trying to unblock with a stick though the valve. By stopping the pressure pump. Why, do you think, did he not do these things ? Ignorance, not aware of the hazards? Pressure from a supervisor to complete the job in time? Not concentrating? (various reasons: worries, tired) Not competent, not trained? Truck driver: How could you have prevented this accident ? Instruct the helper to STOP the job and get assistance from a supervisor. Would you have “seen” the Hazard ? Perhaps yes, perhaps not? How well trained are you? Should you warn a colleague of an obvious Hazard ? Yes always. Better safe than sorry. Supervisor/manager: How could you have prevented this accident ? By ensuring staff is trained/competent, rested and not rushed to complete a task By ensuring the truck is in a good state of maintenance. By having regular toolbox-meetings with the staff to ensure they understand the hazards and appropriate controls. Do you have procedures for these type operations ? You should have. They should be adequate: cover all hazards, simple. Does your staff know + follow these procedures ? You should ensure that they do know the procedures and follow them How do you ensure all Hazards are identified + controlled ? You should have a structured approach for this: HEMP (Hazard and Effect Management Process). Identify, evaluate/access, control, recover. 18/June/2000

Truck left the road down the hill here 10/June Salalah to Thumrait mountain road A truck, transporting liter diesel, came down too fast through this right hand turn. It rolled over and went through the guard rail into the wadi below. 18/June/2000

Should be: low-low when going down hill with heavy load. Brakes malfunctioning? In a hurry to get home (pressure from supervisor?, hungry?) Not concentrating: tired/sleepy, worries Not experienced, not competent Supervisor/manager: How could you have prevented this accident ? Ensure driver is trained/competent and given enough time to do the task Ensure the truck is in perfect state of maintenance How do you ensure the truck is in good condition ? By having competent mechanics that follow the maintenance instructions and intervals as specified by the manufacturer. How do you ensure your driver is trained and competent ? By testing his competence in accredited training courses and by supervisory/comented drives by a more experienced driver. Why would your driver drive too fast ? He thinks he is the best driver in the world and likes going fast You have not given him sufficient time for the journey How do you STOP him driving too fast ? Praise him when he is doing a good job Coach him when he is doing a poor job If repeated violation apply your company discipline system. 18/June/2000

Some learning points from these 4 fatal accidents: - On long open road: warn a lone pedestrian with the horn. He might not have seen you! - A car/truck towards your side of the road: blast your horn, slow down, move off the road. - As a passenger: do not distract the driver. - As a driver: keep concentrated. Tired ?: STOP and rest. - Blockage in pressure system: shut down and get help from a supervisor or qualified operator. - Earthing systems: where provided to eliminate sparks: USE THEM - Driving down a steep slope: engage low-low gear. Go slow!! - Seatbelts save lives: wear them. 18/June/2000
