Roles: PharmD, MD, DVM, Public Health Nurse
What about Ben? Let’s take a look at what we mean by collaborative practice or teamwork in patient care. We’ll observe a care planning meeting of a pharmacist, physician, veterinarian, and public health nurse. During a clinic visit with the physician, it was discovered that the patient had fallen again. This time, the patient stated that she had tripped over her dog. The physician asked the public health nurse to conduct a home evaluation. During the evaluation, the nurse decided to ask that the pharmacist and the veterinarian also visit the patient in herhome. The case starts at the first meeting of the group. They have never talked with one another previously. The Team Meeting The patient, Dora, is a 76-year-old woman with Type 2 diabetes with knee pain and a history of a recent fall. It is suspected that her dog, Ben, is contributing to the risk of falling, though he is also a source of emotional support for Dora. Roles: PharmD, MD, DVM, Public Health Nurse
