CBISA Survey™ hospitals are facilities which are not required to file a Form 990 “Return of Organization Exempt from Income Tax”. CBISA Survey™ hospitals may participate in their state association’s community benefit data collection initiative by entering financial information and narratives into the free CBISA Survey™ software tool. What happens if a hospital coordinator forgets their password? Reset the password by accessing the users page (Options/Org Defaults/Users), select the user, choose “Edit”, enter a new password, then save the record. The Association Rollup Administrator has issued a request for a data snapshot, but the Hospital Coordinator says they can’t answer it. What do I tell them? First, make sure the Coordinator knows where to find the “Rollup” tab (Defaults/Facility, “Edit” the facility, choose the “Rollup tab). If the Coordinator clicks on “Edit” and nothing happens, make sure they have followed the workstation set up guide and that pop-up blockers is turned “off”. If the Coordinator still does not see the “Rollup” tab, call Lyon Software for support. ©Lyon Software 2010

The Hospital Coordinator has accepted the request for a snapshot, but has now informed me that they need to make changes. What do I tell them? Tell the Coordinator to go ahead and make the changes and wait for a new “nag” to answer the request. Then, inform the Association Rollup Administrator to delete the current data snapshot for the facility and generate a second request. The Hospital Coordinator is overwhelmed and asked if more than one person can have access to the program. Can additional Coordinators be added, and if yes, who can add them? Each hospital can have as many Coordinators as necessary. As Survey Administrator, you can add the additional Coordinators, or the current Coordinator may add them (Defaults/Users/Add). ©Lyon Software 2010
