Pharmacy (Week One to Three)
One to one Appointment Education Empowerment Medicine Reconciliation The pharmacist identified pharmaceutical care issues with 18.3% of the prescribed medications 57% of these interventions increased the therapeutic benefit and potentially avoided significant side effects

Overall Results Improved self efficacy over the five week programme and one year period. Magnitude similar to pulmonary rehabilitation. Signal to improved quality of life compared to a historical control group. Qualitative findings demonstrating patient satisfaction and acceptance with the intervention.
