Individual Assistance (IA) Disaster Case Management

Federally funded program administered by the Department of Homeland Security’s Federal Emergency Management Agency (FEMA). In the event of a Presidentially declared disaster, the Governor or Tribal Executive of the impacted State, Territory or Tribal Government may request an option to apply for a DCMP grant if the declaration include Individual Assistance.

5 Disaster Case Management Program
Disaster Case Management is a partnership between a case manager and a disaster survivor. Together, they identify the recovery goals of the survivor and develop a plan to meet those goals. E/L IA Disaster Case Management

7 Disaster Case Management Program
Grant to the State Cooperative Agreement Grant is awarded to the State. The State sub-contracts to service providers. Service providers work cooperatively under the state’s direction to identify survivors and begin case management. Funding awarded to multiple entities which could include the state. Recipients are providers of case management or can sub-contract to additional organizations and begin case management services. E/L IA Disaster Case Management

8 Establish DCMP Agreements
Explore State’s procurement process to develop agreements with potential DCM organizations prior to disaster. Activation of an organization is optional. Recommend a Request for Proposal versus Single Source. Consider organizations ability to hire, train mobilize and potential service areas.

9 Immediate Disaster Case Management (IDCM)
Background Implementation of IDCM started on November 15, 2017. Mission Assignment was issued for Health and Human Services/ Administration of Children and Families. IDCM Services were provided from December 19, 2017 through July 1, 2018, for a total of 15,241 cases. Priorities: Shelters Disaster Recovery Centers (DRCs) Medical Needs Access and Functional Needs Transitional Shelter Assistance (TSA) CONUS and OCONUS Displaced Puerto Rico residents in CONUS E/L IA Disaster Case Management

10 Immediate Disaster Case Management (IDCM)
Background (continued) Special Outreach Efforts Vieques Residents Outreach Culebra Residents Outreach Support to TSA Case Worker Task Force in Puerto Rico Call Out activities to TSA and Max Grants DCM Mailbox Referrals IDCM Mission Assignment extended 60 days through Friday, August 31, This extension is limited to IDCM Hotline activities in support of TSA population in CONUS for Transportation Assistance (which ends on August 30, 2018). 11 Puerto Rico Case Management
In Puerto Rico, there will be multiple awards through a cooperative agreement. The grant application period closed on June 17, 2018. Applications were submitted for Congressional Notification on July 5, 2018. Target date for awards to be issued sometime in July, 2018. E/L IA Disaster Case Management

12 E/L 391 IA Disaster Case Management
Delivery of DCMP Case Managers will work with the survivor to identify holistic recovery gaps/needs. Together with the survivor, the case manager will develop a path to complete recovery. Identified gaps often include medical needs, housing, emotional support, employment, childcare, and other. Case managers work with local recovery groups to identify resources that match the recovery need. E/L IA Disaster Case Management

Timeline The DCMP in Puerto Rico will run for 12 months and could receive an extension based on need. The case manager/survivor relationship will last until the survivor recovery plan is complete or until all resources have been exhausted. E/L IA Disaster Case Management
