1 How to complete a Referral Form via “Student” tab
Referral Forms This PowerPoint will instruct you on how to complete a Referral Form for a Student in Educlimber. After adding a Referral Form, the last two slides will go over how to share the form and add an attachment. How to complete a Referral Form via “Student” tab

2 Logging into EduCLIMBER
Go to website: Step1: Step 2: Enter district ID: 1260 and click How to Log into EduClimber. On the EduClimber home page, you will have the option to Login with a User name and Password OR Login with Google. Click “Login With Google”. It will then prompt you to enter our District ID. Enter 1260 and click Google Sign In.

3 The EduCLIMBER Dashboard
Once logged in, a District Login Message pop-up will display. Click “OK” Now you will see your Dashboard: Click the “Student” tab Once logged in, a District Login Message will pop-up. Please read the message and click OK. You will now see your EduClimber dashboard. On the upper left of the dashboard, Click on the “Student” tab.

4 Selecting a student for the Referral Form
A dropdown menu will appear Begin typing in the first or last name of the student 1 1 2 2 Select the student (highlighted blue) In the “Student” tab, you can begin typing in the first of last name of the student. A dropdown menu will appear with student names as you begin typing. Select the student you want.

5 Adding a Form to Selected Student
After a student is selected, the screen will populate with any information about the student. Select Forms on the left margin After selecting the student, the screen will populate with the student’s information. On the left margin, click on “Forms”. A pop-up window will appear. A pop-up window will appear

6 Adding the Referral Form
Click “ New Form” in the pop-up window. Use the drop down menu from “Form Type” to select the “MTSS Referral Form” + Give your form a Title (Be sure to include the student name in the title) In the pop-up window, click on “+ New Form” tab. Use the drop down in the “Form Type” field to select the “MTSS Referral Form”. Give your form a Title. Be sure to include the student’s name in the title so it is easy to reference. Click when done

7 Filling out the Referral Form
Your form will now show up in your folder of “My Forms”. Double click on the form to fill out the information for the student Fill out the form. When you are finished, click “x” to close the window and the information will be saved. You can also “Print” the form The form will now show up in your folder of “My Forms”. Double click on the form you just created and begin filling out the form. When the form is completed. You can close the window by clicking the “x”. The information on the form will be saved. You may also print your form or share your form. If you want to share the form, click Share. The next slide will show you how to share the form. or “Share” it

8 How to Share the Form Click “x” when done sharing
Click “Share” on the form A “Share Form” pop-up window will appear. Click on “ New” + A dropdown menu will appear. Begin typing in the first or last name to share and select the name (highlighted blue) Click “Update” After clicking “Share”, a “Share Form” window will pop-up. Click on “+ New”. A drop down menu will appear. You can begin typing in the first or last name of the person. A list of names will appear, select the person. Click “Update”. You can repeat this process for others you would like to share the form with and click “update” after each person. Click the “x” on the upper right corner when done. Note: Any forms shared with you can be found when you return to your dashboard in the “My Forms” tile in the “Shared with Me” tab.

9 How to Add an Attachment
Click on “Attachments” on the left margin of the Student Information Page A Student Attachments pop-up window will appear An Upload Attachment pop-up window will appear Give your attachment a description Select a File To add an attachment with the student, click on “Attachments” on the left margin on the student information page. A “Student Attachment” window will pop-up. Click the orange “Upload Attachment” tab. A “Upload Attachment” window will pop-up. Give your attachment a description and select a file to upload. Click “Upload” after your file is selected. Click on “Upload Attachment” Click on “Upload”
