DSRIP Updates Midpoint Assessment Results
The Independent Assessor (IA) for New York’s Delivery System Reform Incentive Payment (DSRIP) Program has completed its Mid-Point Assessment of all twenty-five Performing Provider Systems (PPS). NewYork-Presbyterian/Queens PPS is 1 of the 4 PPSs who received NO formal recommendations from the Independent Assessor (IA) at the midpoint assessment NYPQ PPS PAC Meeting

The NYP/Queens 360 survey sample included 32 participating network organizations identified in the PIT, 20 of those sampled(63%) returned completed survey. Respondents’ answers overall were positive with 79% of all respondents’ answers were either Strongly Agree or “Agree”. The NYPQ aggregate 360 survey score ranked 10th out of 25 PPS NYPQ PPS PAC Meeting

DSRIP Updates Medicaid Accelerated Exchange (MAX) Series
NYP/Queens signed up for the MAX Series which is a Rapid Cycle Continuous Improvement program that brings frontline care providers together to redesign the way care is delivered. The MAX Series will focus on “Targeting Avoidable Hospital Readmissions for High Utilizers* with the goal of reducing 30-day readmissions and overall hospital utilization High Utilizers are defined as any patient that has 4+ Inpatient Admissions in a 12-month timeframe 2 Train-the-Trainer (TTT) participants from PMO will be trained in the RCCI methodology and co-facilitate MAX Series Workshops MAX Action team members include case manager, social workers, nursing rep and attending ED physician from NYP/Queens and the PI staff from Silvercrest SNF NYPQ PPS PAC Meeting

DSRIP Updates Medical Record Review
Independent vendor, Medical Review, appointed by NYSDOH conduct the quality review of 2 measures for every PPS NYP/Queens 2 measures selected for record review Depression Screening Hypertension Control On November 14-16, 2016, the Medical Review team made a site visit and did chart review Pending results, anticipated in January 2017 NYPQ PPS PAC Meeting

PPS Updates IT Updates Phase II RHIO Pilot ongoing
38 partners engaged, 27 pending Of the 38 engaged partners, 12 are connected to RHIO On track of having the partners connected to RHIO by end of DY3 Cureatr Event Notification rollout to SNFs in progress 11 sites engaged; 4 received demos of tool Performance Logic Training & Rollout Project management application is anticipated to go-live in February, 2017. Partners will be trained on how to access and report project task status, using the application – Training (December 2016-January 2017) NYP/Q PPS PAC Meeting

PPS Updates SNF Care Transition, INTERACT and Home Care Projects
The 1st INTERACT Training with Skilled Nursing Facilities was completed on October 24-25 The 2nd session was completed on November 14-15th with our remaining Skilled Nursing Facilities and Home Care partners The SNFs and Home care Partners are currently in the process of implementing the INTERACT tools, starting with SBAR and Stop and Watch MOLST/eMOLST training plan to start from January 2017 NYP/Q PPS PAC Meeting

PPS Updates Palliative Care Project
Congratulations to Dr. Cynthia Pan and Marta Kazandjian, who received an award at the Center to Advance Palliative Care (CAPC) National Palliative Care Conference on October 27, They received the award for the joint Silvercrest - NYP/Queens abstract "New Model: Integrating Palliative Care Across Care Settings." CAPC Needs Assessment tool shared with the LTC partners to outline current practices and identify any gaps Next EPEC Training #6 on December 7, 2016 – modules Goals of Care & Sudden Illness NYP/Q PPS PAC Meeting

Next Steps All PPSs are in Measurement Year 3 where the focus is on Pay-for-Performance and not only Pay-for-Reporting Rapid Cycle Unit development in progress Meeting on 1/17 to review process with key stakeholders PMO team completed the trending, analysis and comparison of previous data sets with the new data set released in MAPP NYP/Q PPS PAC Meeting
