Purpose To inform TSM’s of the health care and mental health care options available to them and their family.

Standards and Practices
Before a TSM separates they should get a physical, a dental check up, and copy of their medical record to include dependents. A through medical evaluation, separation history and physical examination (SHPE) is now required for all TSM’s. This ensures any condition that requires ongoing care is identified and documented. This should ideally begin 180 days prior to separation.

DoD Mental Health Self-Assessment Program
Is a mental health and alcohol screening and referral program provided for families affected by deployment. Program is voluntary and anonymous and offered on-line, by phone, and held at various events on base. AD SVM can self refer to mental health at any MTF. For family members the first eight visits do not require pre-authorization.

National Guard May also seek assistance regarding mental health and substance abuse disorder counseling and treatment resources from the National Guards Bureau’s Psychological Health Program.

Deployed Service Members
Those who have deployed as part of a contingency operation are asked to complete a post-deployment mental health assessments for issues such as depression, PTSD, substance use problems at 6, 12, and 24 months post-redeployment anniversary.

inTransition Confidential coaching program that answers questions about treatment options, provides information about community resources, and appointment with a behavioral health provider at you new location. Any TSM who has been to behavioral health within 1 year of separation from AD are automatically enrolled.

Veterans Affair Coverage
Under the Combat Veteran Authority, the VA offers Combat Vets who were discharged from AD on or after January 28th, 2003 enhanced enrollment placement for 5 years after separation. If you have a condition identified in the SHPE, this may also make you eligible after separation. If you believe you have such conditions file a claim with the VA.

Suicide Resources The Veteran Crisis Line is available for VETS, their families, and friends who may be the 1st to realize a VET is in emotional distress. This is a toll free, confidential resource that connects VETS with qualified caring VA responders.
