Page 4 PROPOSED CHANGES Remove question regarding student participation in Modified High School Assessment, and content area bullets Add( Grades 5, 8 and 10) after Science Alternative assessment

Page 5 PROPOSED CHANGES Change first header to read, “ENGLISH LANGUAGE PROFICIENCY SUMMARY” Change to ACCESS to ELLs question to read, ”What was the student’s performance on the English language proficiency assessment?” Add the word “OR” above the Alternate Access for ELLs question Change the Alternate Access for ELLs question to read, “What was the student’s performance on the alternate English language proficiency assessment? Add a header above the Grades 3-8 Assessments section to read, “PERFORMANCE SUMMARY” Revise MSA table to reflect PARCC assessment in Grades 3-8 in English Language Arts/Literacy, mathematics, and Algebra I (for those students who take Algebra I in middle school) along with 5 proficiency levels Revise Alternate Assessment table to reflect NCSC/MSAA assessment in English Language Arts/Literacy and mathematics along with 5 proficiency levels Delete the Mod-HSA+ column from the HSA Assessments table Revise PARCC High School Assessment table to reflect PARCC assessment in English Language Arts/Literacy, Algebra I, Algebra II and Geometry along with 5 proficiency levels.
