2. Factors influencing engagement
Feeling unprepared for the level of responsibility (isolation, stigmatisation etc.)
Lack of relationship with new clinical team

3. Aim:
Explore and scope the feasibility of a fit for purpose peer supported diabetes self-management programme for young adults (16-25 years)
Approach:
Build stakeholder involvement – local council, employers, education institutions, food academy etc.
‘Normalise’ diabetes management (re-branding i.e. less
clinical)
Create an all-inclusive model for family, friends and carers
Bi-monthly groups within community venues and social settings (Leisure centre, creative-arts centre, bowling, go-karting etc.)

Collaborate with East Ham Community Neighbourhood Team to integrate peer support group into existing youth hub at East Ham Library. Initiative to be launched October 25th
