Malnutrition is a very bad prognostic factor!!! Nutritional assesment
Nutritional risk screening in daily routine; a practible way to become aware of malnutrition in hospital. Since January 2007 we started with a general nutrtitional risk screening when the patients are admitted on our department. This is done interdisciplinary with our nurses. We developed a nutritional score based on the ESPEN guidelines and the guidelines of the AKE the Austrian society of clinical nutrition. To avoid additional expenditure and to decrease time we implement this score in our EDV system for Patient documentation and management (MEDOCS). When the patient is admitted to our department the nurses collect weight, height, nutritional routine and nutritional disturbance and the doctor ask for diseases. This system works with points, which are automatically determined by the computer. In case of malnutrition this is automatically documented and an immediately reaction is possible. Admission to hospital nutritional screening Grazer nutrition score risk patient Nutrional therapy doctor dietitian nursing Nutrional screening in Medocs Nutritional assesment In our patients we had 20% of Malnutrition; 31% female and 68% male. Most of the patients (85%) were between 51 and 80 years old but we had also about 15% patients who were younger than 40 years. This screening is easily done without additional expenditure. General nutritional screening by admittance to the hospital is a very good tool to make aware and to fight against malnutrition. In consequence of this system we developed a perioperative nutritional management for our patients. red arrow= danger green arrow= no danger
