Students already receiving SPED Services
Evaluation Referral Process at a Glance Interventions begin immediately and are captured in eCST. Referrals require a documented meeting (all committee members sign MTSS/RtI Checklist as needed) Students already receiving SPED Services Considering additional testing required to determine changes to support All decisions happen in an ARD meeting with parent input SPED Contact signs the MTSS/RtI Checklist Student documentation of interventions, goals and progress monitoring in SEEDS Students already receiving 504 Services Considering Dyslexia testing or changes to 504 accommodations/plan All decisions happen in a 504 meeting documented in e504 Current evidence of need documented in meeting notes Parent is invited to 504 meeting for input MTSS/RtI Checklist can be used as determined by 504 committee Accommodation changes occur immediately with parent notification of changes Considering additional testing for SPED services All decisions occur during a 504 Meeting documented in e504 Student is continuing to struggle despite 504 accommodations Documentation of Intervention, goals and progress monitoring in e504 504 Coordinator signs MTSS/RtI Checklist Notice of decision given within 15 days by 504 Coordinator to parent Informed consent given to parent by LSSP Verbal or written consent for testing from parent is not given before a LSSP has reviewed the folder (5 day window for LSSP to review) General Education Students not receiving 504 or SPED Services Considering 504/Dyslexia Support Determined during a CST meeting (meeting notes in eCST) with parent input Parent Requests and outside evaluations are automatic referrals Dyslexia Checklist needed for all Dyslexia referrals MTSS/RtI Checklist signed by 504 Coordinator Folder is given to 504 Coordinator by CST Chair Considering testing for SPED services Determined during a CST Meeting (meeting notes in eCST) with parent input Parent Requests and outside evaluations are considered (not automatic referral) Speech Checklist is needed to determine language needs MTSS/RtI Checklist is signed by CST committee Notice of decision is given to parent within 15 days, consent for testing is not given until after notice of decision has been made. Folder is given to SEES Staff by CST Chair
