Improving Health Needs Assessment processes for newly looked after children & young people. The title for my poster is Improving Health Needs Assessment Processes for newly looked after children and young people. I was the only AHP and clinical person involved in this team.

QI Practitioners Craig Morris, Project Manager NES Resources 3 Quality Ambitions Leading Better Care Aims LEADERSHIP AND MANAGEMENT The theme was: Leadership & QI This was my project for the QI Practitioners Course, run in Fife by Craig Morris. I have highlighted a range of resources that I used to help me with this work.

4 0 Strategic priority 100% children who become looked after should receive a health assessment within 28 days (4 weeks) NHS Fife Children & Young People’s Strategy Corporate Parent Board and Action Plan 2015 AIM Background Looked after children and young people have poorer life outcomes and are likely to have additional physical and emotional health needs compared with their non looked after peers A Health Needs Assessment (HA) should be provided for all children newly looked after, within 28 days of notification from a Council to a Health Board, enabling earlier planned access to any required services Aims NHS Fife Corporate Parent Board approved a target to increase from 60 to 80% newly looked after children who are offered a Health Needs Assessment within 4 weeks of notification from Social Work, by 31 March 2016 To do this by 1) review Social Work and NHS processes 2) work with key staff to identify effective ways of working, to streamline systems, increase NHS capacity and make more effective use of all partners resources ↑ to 80% children who become looked after have a health assessment within 28 days (Currently 60%) when all paperwork is received from Council.

5 Methods Agreed what we were trying to do! Detailed Process Mapping
Accurate, reliable, validated data Data for improvement, not data for performance Collaboration – identify key stakeholders partnership working – awareness Methods Quality improvement methodology including detailed mapping of operational processes and Leading Better Care Aims principles were used to direct the performance of various teams, working in partnership to promote collaborative discussions across staff groups in both organisations. This helped to facilitate focused problem solving on selected topics and communicate consistent messages about progress, using data for improvement and data for performance.

6 Impact The tools work! The Impact on outcomes for this project was very positive I hope you will find time to look at my poster and our results. Fife Corporate Parent Board has made measurable progress in delivering against this strategic aim. I would very much like to make contact with anyone else working in this field, to exchange ideas and current performance data.
