Survey links by facility
Due to the number of departments, we will be shifting to another survey platform that better meets our needs and will need to break the survey out into separate survey links by facility (Clinics, Morrilton, Hot Springs, North, Infirmary and Enterprise)
If you work with multiple facilities, please ensure you take the survey for each facility you will be rating
