Get services and supports they need in a timely and cost-effective manner
In settings they want from whom they want
And if needs change, services and supports change accordingly

3. Ohio HOME Choice
Is a wrap around program in addition to Ohio’s existing infrastructure of long-term services and supports
It is not a waiver program
Offers “extra” services to individuals for the first 365 days post discharge
Supports the new Home and Community Based Services rules and Ohio’s balancing efforts

9. Benefits of HOME Choice
Transition Coordinator to help before and after discharge
Up to $500 in transportation funds to assist family with hospital trainings and some appointments during hospitalization and 30 days post discharge
Up to $2,000 in goods and services funding to assist with housing and other needs within guidelines
Case Manager assigned for duration of program to those not receiving waiver services

10. Who is eligible? To participate in HOME Choice individuals must:
Have lived in a facility-based (NF, ICF-IID, Hospital or RTF for children) care setting for a least 90 days,
Have care needs evaluated by HOME Choice staff,
Qualify for Medicaid, and
Move into qualified housing

11. Qualified Housing The following are types of qualified housing:
A home owned or leased by the individual or family member
An individually leased and lockable apartment rented by the individual or family member
Sleeping, bathing, and cooking areas within the unit over which the individual or the individual’s family has domain and control
A community-based residential setting with no more than four unrelated persons

Individuals:
Express an interest in HOME Choice
Complete an application and submit to HOME Choice Operations (ODM)
Are assigned to a Pre-Transition Case Management Agency
Provide current contact information for family

Question: When do I send in the application?
Answer: At any point during the admission, if you anticipate a 90 day stay.

Question: Can HOME Choice work with children moving into the foster system or who are under county custody?
Answer: Yes, with agreement from the public children services agency.

Pre-Transition Case Manager
Meets face-to-face with family
Makes eligibility recommendations
Completes Community Readiness Tool
Ensures the Informed Consent is signed
Supports family in choosing Transition Coordinator
Participates in discharge planning with hospital & the individual

17. Referrals from Transition Coordination Agencies
Transition Coordination Agency refers applicant to HOME Choice:
Sends in all PTCM documents instead of PTCM agency
Assigned as Transition Coordinator
Continue to work with child/youth and assist with transition to the community
Paid all deliverables as documents are submitted to HOME Choice

Transition Coordinator
Assists in securing housing that is safe and appropriate
Identifies necessary resources
Provides benefits coordination
Assists with planning a budget, if needed
Provides service linkage for a successful community transition
Continues 90 days post-discharge

Household Start-Up Items
Clothing
Birth Certificates and Identification

20. What Happens AFTER Discharge?
Individual is enrolled in Home and Community-Based Services for which they are eligible and meets their needs
If enrolled on waiver, waiver service coordinator authorizes and coordinates HOME Choice services during the 365 days post discharge
Waiver service coordinator reports certain changes in status to HOME Choice Operations

21. HOME Choice Case Manager
Available to participants who do not enroll on a waiver, and receive State Plan Medicaid services
Authorizes and coordinates HOME Choice Services post discharge for the 365 days enrolled on HOME Choice
Assists with community resources and maintenance of benefits
