Start Physician at Healthcare Facility logs in to DOH SL attestation process using provided link Physician navigates to Sick Leave Attestation form Sick Leave Attestation request filled in / retrieved by Physician Form complete ? Yes Submit form to DOH for processing SL attestation request Save form in Draft mode for future submission No DOH Medical Board Review process Provide additional details requested by DOH to support SL attestation request Print Certificate, stamp & sign and deliver to patient Request Approved by DOH ? Yes No Request Rejected End

Attendant Sick leave To add attendant details, follow the below steps:
Click on “Add Attendant” button as highlighted in below screenshot Enter the attendant details in “Attendant Information” section. Maximum of 2 attendants could be added to the system.

Next Steps… We expect Physicians (Users) who will submit Sick Leave Attestation requests to register and acquire user ID’s at DOH will promptly authorize usernames and passwords to all valid requests from users, such activated user ID’s will receive an confirming activation DOH IT Help Desk / Support contacts: BPA Support ( ) or
