SEPARATION HISTORY AND PHYSICAL EXAMINATION (SHPE)
Documents and assesses medical history Medical concerns during military career Current health status Identify and document any medical conditions that require ongoing care

Mental health and alcohol screening and referral program for military families and service members affected by deployment and mobilization Voluntary and anonymous

Offered online, by phone and through special events at installations and reserve units Individualized results and military health resources are provided at the end of every assessment

ACTIVE DUTY: must be seen at Medical Treatment Facility AD FAMILY MEMBERS: first eight visits do not require preauthorization if the family member seeks care within the TRICARE network

AUTOMATICALLY ENROLLED if service member has seen behavioral health provider within one year of separation from AD Ability to OPT-OUT when contacted by coach

Combat Veteran Authority
ELIGIBILITY: Served on AD in theater of combat operations after November 11, 1998 and have been discharged under other than dishonorable conditions Discharged from AD on or after January 28, 2003 5 year enrollment period begins on discharge date

Substance use disorders Service members with urgent mental health or substance use are considered high priority for VA care
