IMPORTANT: If you check “Observation,” you cannot uncheck it. Likewise, with mid-semester and final evaluations. If you need to make changes, you can “decline to sign” and launch a new document. See the final slide for instructions. Clicking ”Observation” brings up fields for the date, start, and stop times. These are required, along with the observation form as an attachment.

5 Indicate if debriefing or planning occurred. Or both.
Paper clip icons go from yellow to gray once a document is attached. If you select either the Mid-Semester or the Final Evaluation, you will attach that evaluation form. If you also selected observation, you will attach 2 documents, one for each paperclip icon. You can attach multiple documents for each paperclip; however, each paperclip requires at least one document.

6 Once you sign, you’ll see an option to download or print your document.
WAIT! You’ve finished signing, but you still need a signature from the UTeach student. Upload the observation into Box only when it’s been signed by the student. When that happens, you’ll get an indicating that the document has been signed by everyone.

7 Click to view. The access code is required.
Select “Combined PDF” from the download menu. This is a combination of the observation form, any attachments, and the certificate that logs all of the steps (down to the GPS!). Please save with the following naming convention: EID_ROLE_DATE_documentslist EX: als24_UF_ _FocusedContent In the example, the student’s EID is followed by the role of the observer, then the date – Sep. 17, 2017 – and then the type of documentation. Please be sure to use all EIGHT digits to represent the date MMDDYYYY. See “File Naming Convention” for the Documents Lists. This naming convention helps us meet TEA reporting requirements. Make a mistake? It’s OK. You can always start over. Depending on where you are in the process, you’ll see ”Other Actions” as an option at the top right or bottom right of the page. Select “Decline to Sign” and then “Continue.” Enter a reason and “Decline to Sign” (again). Use the link to launch a fresh document.
