Click the link above to launch the weekly log.
Once you complete the Signer Information, you’ll need the access code below to continue.

3. When you have filled in all of the information, you’ll be prompted to initial the total and sign and then to fill in the name of your school.

4. Once you sign, you’ll see an option to download or print your document.
WAIT!
You’ve finished signing, but you still need a signature from your cooperating teaching.
Upload the weekly log into Box only when it’s been signed by your cooperating teacher. When that happens, you’ll get an indicating that the document has been signed by everyone.
All parties have completed AT Weekly log

5. Click to view. The access code you were given in the seminar is required.
Select “Combined PDF” from the download menu. This is a combination of the weekly log and the certificate that logs all of the steps (down to the GPS!).
Please save with the following naming convention:
EID_log_MMDDYYY
MMDDYY is the FIRST DAY of the work week (MONDAY). For example:
EID_log_

6. Upload the completed Weekly Log into your folder on UT Box.
The document title will be: EID_log_MMDDYYYY.
This naming convention allows quick sorting by document type and by week.

Make a mistake? It’s OK. You can always start over. Depending on where you are in the process, you’ll see ”Other Actions” as an option at the top right or bottom right of the page.
Select “Decline to Sign” and then “Continue.”
Enter a reason and “Decline to Sign” (again).
Use the link to launch a fresh document.
