Developmental Promotion, Early Detection and Linkage
This set of measures describes a complete system of home visiting services that deliver developmental promotion, early detection of developmental risks in partnership with families, and linkage to services. They are intended to help guide quality improvement efforts and to support data-driven decision-making in practice. From 2013 to 2016, the measures underwent several iterations of testing and refinement by a group of early adopters: home visitors and supervisors in local implementing agencies, MIECHV grantees and expert faculty. Several resources have been developed to facilitate their use, including:

2. % of children with positive screen for developmental risk/delay
SMART AIM: 80% of children with a developmental or behavioral concern identified in partnership with the family  will receive targeted developmental promotion in a timely manner, including an appropriate combination of home visitor-delivered developmental promotion, community services and/or Part B/C services

The measures include outcome and process measures.
The outcome measures show progress toward the SMART AIM:
% of children with a developmental or behavioral concern identified in partnership with the family  will receive targeted developmental promotion in a timely manner, including an appropriate combination of home visitor-delivered developmental promotion, community services and/or Part B/C services
The process measures show how well each primary driver is working. The primary drivers are essential pieces of the system that need to be in place and working well in order to reach the SMART Aim.
For Primary Driver 1 (Standardized and reliable processes for developmental and behavioral surveillance and screening):
% of home visits where parents are asked if they have any concerns about child’s development, behavior or learning
% of children will be screened with appropriate instrument at least every 6 months
% of children with positive screen for developmental risk/delay
For Primary Driver 2 (Reliable and effective systems for referral and follow-up):
% children referred to community services who received services within 30 days
% of children with a developmental or behavioral concern appropriate for referral to Part B/C services that were referred within 7 days
