1 Developmental Promotion, Early Detection and Linkage
HV CoIIN Measures Developmental Promotion, Early Detection and Linkage This set of measures describes a complete system of home visiting services that deliver developmental promotion, early detection of developmental risks in partnership with families, and linkage to services. They are intended to help guide quality improvement efforts and to support data-driven decision-making in practice. From 2013 to 2016, the measures underwent several iterations of testing and refinement by a group of early adopters: home visitors and supervisors in local implementing agencies, MIECHV grantees and expert faculty. Several resources have been developed to facilitate their use, including: This orientation powerpoint A Cheat Sheet that lists all of the measures with their numerators and denominators An excel data reporting template, “XX,” with data collection tools and automated run charts An instruction document with detailed instructions and operational definitions to help you complete the reporting template

2 % of children with positive screen for developmental risk/delay
SMART AIM: 80% of children with a developmental or behavioral concern identified in partnership with the family  will receive targeted developmental promotion in a timely manner, including an appropriate combination of home visitor-delivered developmental promotion, community services and/or Part B/C services Primary Driver 1 Standardized and reliable processes for developmental and behavioral surveillance and screening. % of home visits where parents are asked if they have any concerns about child’s development, behavior or learning % of children screened for developmental risk/delay within the last 6 months % of children with positive screen for developmental risk/delay Primary Driver 2 Reliable and effective systems for referral and follow-up % children referred to community services who received services within 30 days % of children with a developmental or behavioral concern appropriate for referral to Part B/C services that were referred within 7 days % of children referred to Part B/C services who receive IFSP planning meeting within 45 days % of children referred to Part B/C services for whom Home visitor knows the status of referral Primary Driver 3 Capacity of and support for home visitors to address development in the target population % of children with developmental or behavioral concerns who receive Home Visitor-provided individualized, targeted developmental promotion by next visit Primary Driver 4 Families engaged in promotion of healthy development The measures include outcome and process measures. The outcome measures show progress toward the SMART AIM: % of children with a developmental or behavioral concern identified in partnership with the family  will receive targeted developmental promotion in a timely manner, including an appropriate combination of home visitor-delivered developmental promotion, community services and/or Part B/C services The process measures show how well each primary driver is working. The primary drivers are essential pieces of the system that need to be in place and working well in order to reach the SMART Aim. For Primary Driver 1 (Standardized and reliable processes for developmental and behavioral surveillance and screening): % of home visits where parents are asked if they have any concerns about child’s development, behavior or learning % of children will be screened with appropriate instrument at least every 6 months % of children with positive screen for developmental risk/delay For Primary Driver 2 (Reliable and effective systems for referral and follow-up): % children referred to community services who received services within 30 days % of children with a developmental or behavioral concern appropriate for referral to Part B/C services that were referred within 7 days % of children referred to Part B/C services who receive IFSP planning meeting within 45 days % of children referred to Part B/C services for whom Home visitor knows the status of referral For Primary Driver 3 (Capacity of and support for home visitors to address development in the target population) % of children with developmental or behavioral concerns who receive Home Visitor-provided individualized, targeted developmental promotion by next visit For Primary Driver 5 Data system for tracking surveillance & screening periodicity, results, referral, acceptance of referral & receipt of service % of team members using CQI data in practice Primary Driver 5 Data system for tracking surveillance & screening periodicity, results, referral, acceptance of referral & receipt of service % of team members using CQI data in practice Education Development Center Inc. 3 HV CoIIN DevPEDI Report 2015-2016 Color coded by Primary Driver
When viewed on a single page, these measures provide a visual summary of the entire system of support for developmental promotion, early detection of developmental risks in partnership with families, and linkage to services. The white run charts show the outcome measure over time. The colored run charts who the process measures over time. 4 HV CoIIN Measures were selected to capture steps in the process of Developmental Promotion, Early Detection and Linkage My First Template Ongoing, regular surveillance and screening of children’s development Children with developmental risk identified are referred to services appropriately Children with developmental risk identified receive an appropriate combination of services in a timely way % of home visits this month where parents were asked if they have concerns regarding child’s development, behavior or learning % of children with developmental or behavioral concerns who receive Home Visitor-provided targeted developmental promotion by next visit % of children with positive screen for developmental risk/delay % of children referred to community services who received services within 30 days % of children with a developmental or behavioral concern appropriate for referral to Part B/C services that were referred within 7 days % of children screened for developmental risk/delay within the last 6 months % of children referred to Part B/C services who receive Part B/C planning meeting for IFSP within 45 days of referral Another way to think about the measures is that they reflect steps in the process of developmental promotion, early detection of developmental risks in partnership with families, and linkage to services. This slide presents a simplified process map. Above the brown line, each box shows a step in the process along the way to developmental promotion, early detection of developmental risks in partnership with families, and linkage to services. The boxes below the brown line contain the measure(s) that align with each step in the process. % of children referred to Part B/C services for whom Home visitor knows the status of referral % of children with a developmental or behavioral concern identified in partnership with the family that received an appropriate combination of promotion and services
