2. AOB

AOB As the case for CTS( Jan 2015) could be discussed in conjunction with Feb 2015 case in the next review meeting, the time in next meeting is expected to be longer. Chairman has also invited the CTS coordinator to come earlier in the next meeting. An new form of checklist submission for ‘NON-COMPLICATION CASE’ will also be accepted in the coming month, A general case-list ( sample attached in the next page) will be sent to coordinator 1-2 weeks prior to the meeting. For ‘NON-COMPLICATION CASE’, please ensure you have confirmed these case were non-complication by the return the ‘general case-list’ or ‘complication checklist’(the former form). For complication case, please ensure you have submitted all relevant checklist. Coordinator please chase medical record of the case without discharge summary (died in ICU).
