Session Two: Roles and Goals Learning Objectives
Understand how team roles are differentiated. Understand, “Transdisciplinary,” and what it looks like. Learn tools that clarify how team roles can work together. Understand the quality compass for team goals.

What is a team? Interdisciplinary Multidisciplinary Interdependent We need a lexicon Within this room there are multiple disciplines represented, does that mean we’re a team? A cooperative venture that requires them to work in parallel to contribute to care but don’t require them to interact or necessarily communicate. There are smaller groups that are meeting around a common purpose or task and so at the very basic level requires them to communicate in a way that makes them interdisciplinary. However, a true functioning, high-performing team becomes interdependent or TRANSDISCIPLINARY. Transdisciplinary: A strategy that crosses many disciplinary boundaries to create a holistic approach.

Everyone knows everyone’s roles Team members can leverage from each other. Team members learn from each other, i.e. their individual practices become transdisciplinary. Team based care can increases job satisfaction. Integrated care is not a person, i.e. the BHC. Knows other’s roles means the ability to describe it to other’s Leverage examples: a warm handoff can transfer leverage the relationship, trust and confidence from one provider to another. Team members can reinforce or clarify information and messaging. Teams members can bring new information or perspectives , so the patients can be better understood. Team satisfaction: Feeling supported, learn from one another, less stress, enjoy one another, higher quality of life.

Do they accurately reflect the team role? Are they flexible enough for transdisciplinary practice? Do they explicitly address team responsibilities? Are they discussed & understood by the team? Are team accountabilities discussed during performance evaluations?
