2. How access your patient Timeline report
What is the Individual Patient Timeline report
Health Board/Trust users will be able to access and view an individual patient Timeline Report as an added bonus feature when using the Epilepsy12 data platform. N.B. All patient data displayed within this guide is fictional.
The individual patient timeline report provides a summary of dates and information for the following data entered within the clinical audit forms on the data platform:
Outcome
Displays the date of the last seizure and when this was recorded by the clinical team based on the data entered into the outcome tab on the clinical audit forms.
Care
planning
Displays key Care Planning dates & information as entered into the Care Planning tab on the clinical audit forms
Displays details of discussion of First aid, risk, service contact details, water safety, valproate, SUDEP and more.
Investigations
Displays dates of when MRI, ECG first EEG were obtained from within the investigations tab of the clinical audit forms.
Treatment
Displays, for each AED and Rescue Medication prescribed, a graphic showing date started and date stopped/ongoing. Graphic displays drug name and dates from the treatment tab of the clinical audit forms.
DESSCRIBE
Shows key dates from Description of episodes tab dataset (e.g. same as RTF report) etc.

Step 1
There are two ways to access a patient’s Individual Timeline report. Option 1 is to select the ‘Reports’ tab on the Home Page.

Step 2
Using the drop down options under By case ID, select the name of a patient and click the computer icon to view the Timeline report.

Step 3
The Patient Timeline report can also be accessed directly via a patient’s clinical audit form (see user guide - accessing the clinical audit form). Click the ‘Time Line Report’ button to view the report.

Step 5
The Timeline report displays summaries of data entered for the patient within their respective clinical audit forms.
Hovering over the red lines generates pop out summaries of captured information.
