Architecturally, the glands are small with slightly amphophilic cytoplasm, but lack cytological atypia, and some are atrophic (arrows). Also, as the glands are on the edge of the gland, one cannot exclude with certainty that they are not outpouchings off adjacent HGPIN or carcinoma that is not represented on the core. Consequently, the risk of cancer following atypical glands on prostate biopsy is ∼40%, which is twofold higher than that following a benign biopsy. Tosoian, J. J. et al.
