1 Community Project By Amanda Cooley

2 Goal To increase the knowledge in the community of Iron County about heart disease and modifiable risk factors that lead to coronary artery disease.

3 Healthy people 2020 Objective to be met.
To reduce coronary artery disease

4 Impact objective To reduce coronary artery disease and the mortality rate of heart disease in Iron County through education to the community. Education is one of our greatest tools as health care providers, but unless the education is being put out into the community before the occurance of disease it is not as effective. I hope to have some impact on my community and increase their knowledge about heart disease.

5 Outcome objective The year measuring 2015 by the Utah Indicator-Based Information System of Public Health in Iron County will have a decreased cause of death by heart disease by at least 1%. (Query Results for Mortality ICD-10 Query Module for Utah Counties and Local Health Districts, 2014) The 2013 measurement for number of deaths caused by heart disease was 3,291 with a total population of 28,857.

6 Community based intervention
My proposed community based intervention is to increase the communities knowledge about modifiable risk factors that put them at risk for heart disease. In doing so I want to provide education via handouts to be displayed and given out in community resource areas as well as public areas through out the community.

7 Heart disease Heart disease is the number one cause of death in the industrialized world. (Agewall, 2012) Modifiable risk factors include smoking, abnormal lipids, hypertension, diabetes, abdominal obesity, some psychosocial factors, poor diet, poor physical activity, and alcohol consumption. (Agewall, 2012) Public Health Education is one way to bring awareness and help reduce modifiable risk factors. (Agewall, 2012)

I chose this particular poster because it was available for public distribution and because not only did it include the information I wanted the community to have it provided additional resources for information on heart disease. (Heart Disease, 2014) I printed these double-sided in color and distributed them to 5 places in the community. One hundred handouts were provided at each location with a total of 500 handouts.

10 Distributing I provided 100 hundred handouts to the following community buildings: Cedar City Aquatic Center, Southwest Behavioral Health Center, Southwest Applied Technology College, Cedar Ridge Family Medicine, and Southern Utah University student resources center. All 5 places of distribution were asked permission and gladly accepted to display handouts for the community .

11 Distributed to areas around the community
Distributed to the Southwest Behavioral Health Center. Community members can get assistance with behavioral health here. Distributed to the Aquatic Center which more of a community center where members of the community can to go to exercise , swim, and attend classes.

12 Distributed to areas around the community
Distributed to the SWATC where certified health programs are taught. Paige is on the left. She is a wonderful CNA that got certified at the SWATC and currently works with me.
