IEP/EP was not available at the time of the Audit Matrix was not available at the time of the Audit IEP/EP did not have General Education Teacher’s Signature IEP/EP was expired Cost Factor (FEFP) in the Matrix did not match FTE Funding (DECO) Instructional Minutes reported Incorrectly

Most Common Mistakes that cause Audit Findings
IEP/EP Meetings for expiring plans are not held in time for FTE Survey period. IEP and/or Matrix are not locked in ESE-EMS in time for rollover into DSIS for DECO/FTE processing DECO is not updated for records that do not match the current IEP/Matrix Matrix event is not done and locked after for IEP meeting Signature pages are not scanned and uploaded into ESE-EMS General Education teacher does not sign the IEP/EP Therapies are not scheduled in DSIS correctly ESE records for students returning from Hospital Homebound are not updated correctly. DECO was not compared to DSIS to make sure all students were accounted for and all data was correct.

ESE STUDENT SCHEDULING
Students need to be scheduled into the correct educational setting per the IEP. There are three Types of Placements (Settings): ESE class (Resource) Pure ESE Class taught by ESE Teacher General Education Class with Support (Inclusion) Co-Teach = General Education teacher & ESE Teacher teach together all class period. Both teachers assigned in DSIS. Support Facilitation = General Education Teacher is main teacher, ESE teacher visits class for part of the class period to work with students (frequency determined by IEP). Only General Education Teacher is assigned in DSIS Consultation – General Education Teacher is the main teacher, ESE teacher provides external support to teacher General Education Class without Support (GE)

ESE TEACHER & PARA 2018-2019 ALLOCATIONS
May & July CASAS will be the same as September’s CASAS. Changes in enrollment & scheduling will not be reflected until August 2018 ESE program resource teachers are generated on CASAS based on the FTE generated from direct contact hours documented on PF4. PF4 contact hours are generated based on what courses a student is scheduled into on the PF1 screen. Consultation ESE teachers are generated on CASAS based on the students that have a “Y” documented on the PF17 screen Classroom paras are generated on CASAS based on the FTE generated from direct contact hours documented on PF4. Classroom paras are not generated on a 1 per classroom basis. Ratios are in the allocation handbook.

2018-2019 ALLOCATIONS continued
After the August roll over, make sure all courses are coded correctly in DSIS. Pull Portal and File Download Manager ESE reports to ensure that all ESE students are scheduled and IEPs are current Updated ESE Allocation Reports are available in Control-D in August after the roll over. CASAS will be generated based on what is in DSIS as of Friday, September 14, Since ESE-EMS has a hour data upload window, ESE IEP changes need to be done by Wednesday, September 12, 2018
