Suggestions Hand written with team input
Begin with direct expression of sorry and use the word death Focus on the sadness of the death and not on clinical details (also avoids legal issues) Content considerations: Personal memory of patient or family Comfort patient received from loved ones Courage that the patient showed Privilege to have participated in the patient’s care Conclude with words of support. Avoid superficial statements (“It was meant to be”, “I know how you feel”)

It is with great sadness that I write to you on behalf of my entire medical team to send you our condolences on the death of your daughter, Julie. We were inspired by her concern for your well-being and her ability to make us all smile with her stories about camping. We also saw the comfort that she derived from your presence at her bedside.
