6 Key Partners in a broad collaboration should include:
State and community partnerships Support organizations Grant-makers and government agencies

7 A Model for Community Change:
Community context and planning. Community action and intervention. Community and system change. Risk and protective factors and widespread behavior change. Improving more distant outcomes.

8 Community context is influenced by:
Hopes and expectations. Job and family demands. Problems. Financial resources. The community’s reaction. The broader political and social context.

9 Community Change: Developing a new program (or modifying an existing one), bringing about a change in policy, or adjusting a practice related to the group’s mission.

10 Affect community health and development through:
Targeted mission. Action planning. Changes in leadership. Resources for community mobilizers. Documentation and feedback. Technical assistance. Making outcomes matter.

11 Mission statement: Your organization’s statement of purpose.

12 Action planning: Identifying specific changes in the community or broader system, and implementing related action steps to bring them about.
