The Primary Care Almanac
Just as a reminder, you can access your Almanac from the tools menu in CPRS (It may be listed differently than you see here) or by logging into VSSC at VSSC.med.va.gov. It is crucial that that staff be knowledgeable on how to access this information. Just as important as it is to Identify patients with uncontrolled medical issues at higher risk for adverse outcomes ow to use this information to improve their care with additional resources. In addition, we need to work to restructure the care we give from being reactive and defensive system to the model that is proactive in the care we deliver to patients. PACTs can bring care coordination to the forefront by promoting discussions of this during huddles and care planning sessions. There is an enormous amount of information available in the Primary Care Almanac, and a complete discussion here is not practical. But we would like to try to highlight some things that are available to the Primary Care teams. Or ...

If you export to an excel spreadsheet you can add notes and print to use during your weekly scrubbing or huddle time Stratification Tool for Opioid Risk Mitigation (STORM) Dashboard The STORM score predicts the 1 year specific risk of a suicide-related event or opioid, sedative, or acetaminophen poisoning/overdose MAY 2018

If you click on the patients name it will take you to this screen. This is the info from the previous screen and more. Here you will see Demographic info, next of Kin, housing status, service and service connection, last visit info, are they using SM… Team assignment Last vitals risk scores for this patient. CAN (Care Assessment Needs Score), NOSOS (risk score), ACSC (Ambulatory Care Sensitive Conditions) Immunizations Are they a candidate for HT Goals of care conv – not all facilities are participating in this yet MAY 2018

Overview of VA PACT University Why We Are Here VA PACT University (VPU) has been established to provide an online educational experience that will enable PACT staff to engage in personal and professional development that will ensure competency in the principles of the patient centered medical home. VPU is for PACT staff and primary care leaders at all levels. Our Goal The goal is to share resources and provide training that will help PACT Staff improve their knowledge, skills, and abilities related to foundational and advanced PACT concepts. Staff will be equipped to function at the top of their scope of practice through completing all four levels of VPU curriculum. Our Curriculum The VPU curriculum has been designed to provide occupation-specific content that will bridge the learner from a novice to expert in primary care practice. Graduates of VPU will help lead the VA forward in ensuring Veterans receive personalized, proactive, patient driven primary care.
