Reserved appointments for School, Sport, and CYSS physicals from 6-17 August (not Wed, 8AUG)
Tips to make it a smooth process:
1) Arrive 20 min early to complete paperwork and screening.
2) Bring copies of your child’s immunization record or yellow shot record (if possible).
3) Does your child wear glasses? Bring the glasses to the appointment.
4) Please have the parent’s portion of the forms completed prior to arrival.
The PA school forms can be found online; search “Date of Exam – PA DOH”
The CYS Health form can be found online; search “CYS HASPS”
5) A form will be needed for EACH physical (School, Sport, CYS, School Medication Administration form).
